I regret to inform you that, as of September 1 2019, I will no longer be able to continue as your family physician. My husband is in the Canadian Forces and we have been posted out of the area and will be relocating.
The Caroline Family Health Team (FHT) is currently finalizing arrangements for another physician to assume your care. For any current medical concerns you can continue to see my locum, Dr. Hira Jamshaid, until June 30, 2019. I will be returning for the months of July and August 2019, with your new physician being in place for transition of care starting September 1, 2019. Please note that at this time none of the current physicians at the Caroline FHT will be able to accommodate requests for transfers to their existing practices.
There will be no gaps in care during this transition period. If you have an urgent issue you can continue to call our reception to request an appointment to be seen during normal or extended clinic hours. If you have an emergency you are advised to go to the emergency room at your closest hospital.
If you do not wish to continue to receive care from the Caroline FHT, you may contact our office to request to have your medical records transferred to a physician at another clinic. If you will be continuing to receive care at the Caroline FHT, your record will automatically be transferred to the new physician, and no action is required on your end. If you would like further details about the new physician, please feel free to call the office after July 1, or to see me prior to my departure.
I apologize for the inconvenience that this may cause you and your family. My husband and I were very much hoping to stay in Burlington; unfortunately, this wasn’t a possibility.
