The Diabetes Management Program provides enhanced diabetes care to improve health outcomes through provision of a structured plan of care and case management approach.
The INR Management Program has been instituted to support our patients who use pharmaceutical anti-coagulants to aid in the prevention of blood clot formation.
Well Baby Care
The Well Baby Program structures our care and monitoring of child development (ages birth – 3 years) and supports effective parents as the foundation of child wellness.
The Clinical Pharmacy Program provides continuous access to the expert knowledge of a Clinical Pharmacist through direct collaboration with family physicians and the interdisciplinary team.
The Prenatal Care Program is provided by both Nurse Practitioner and Registered Nurse staff as an integral component of primary obstetrical care provided by three C-FHT physicians.
Registered Dietitians (RDs) are regulated health professionals who are uniquely trained to advise on diet, food and nutrition.
Lung Health Clinic
A nurse who is a Certified Respiratory Educator provides lung health services for our patients.
Occupational Therapy Program
The Occupational Therapy (OT) Program provides support to seniors (65+) by collaboratively working with you.
The Immunization Clinic is available for our patients only on a “walk-in” basis every Tuesday from 8:30am to 11:30am and 2:00pm to 4:30pm.
The Heart Health Program provides disease management support to patients have complex needs related to cardiovascular health.
