Adult Mental Health

Adult Mental Health. Adult Mental Health.
The Adult Mental Health Program provides support to patients who are identified to have mental health needs.  Assessment and interventions are provided by a team of social workers, a clinical pharmacist, with leadership provided by a FHT physician with expert knowledge in mental health and consultation provided by a sessional psychiatrist.

Program objectives include:
  • To assess the mental health status and support needs of all patients referred to the program by a C-FHT physician or interdisciplinary team member
  • To establish an appropriate plan of care for patients registered to the program
  • To promote consideration of mental health needs as a standard of practice at the C-FHT

The Adult Mental Health Program provides the following services to patients within the Caroline Family Health Team:
  • Clinical assessment
  • Treatment (including but not limited to Cognitive Behavioural Therapy and pharmacologic therapy)
  • Disease support
  • Education
  • Referral to community services
  • Ongoing care and support for patients with persistent illness to prevent deterioration and potential hospitalization
