REGISTERED DIETITIAN

 

Recognized as a leader in providing innovative primary health care, we are seeking a dynamic individual to join the multi-disciplinary team at Caroline Family Health Team.  The Registered Dietitian will provide patient assessment and consultation, direct service, facilitation of referrals to community services, and will function as a resource to the physicians and interdisciplinary team members. 

 

The successful candidate requires:

• Current registration with the College of Dietitians of Ontario

• Have obtained or working towards Certified Diabetes Educator

• B.Sc. degree in Nutrition

• Minimum 2 plus years work experience in nutrition preferably Diabetes Education

• Demonstrated skills in the provision of clinical and educational services to individuals and  groups
  

• Knowledge of community resources within Burlington and area

• Demonstrated ability to work as a member of an interdisciplinary team

• Excellent communication skills, both verbal and written
